We provide personalized, physician-led psychiatric care for adults seeking diagnostic clarity, comprehensive evaluation, medication management, treatment-resistant depression consultations, and interventional psychiatry services.

Appointment Requests and Confirmation
Appointments are available by request and must be confirmed by our office. After submitting your request, you will receive intake forms, consent documents, insurance information requests, and payment authorization forms by text message and/or email.
Your appointment is not considered confirmed until all required information has been completed and you have received confirmation.
All consultations are by appointment only. Walk-ins are not available.
New Patient Forms
New patients must complete their intake paperwork before the appointment. These forms allow your provider to review your symptoms, treatment history, medications, medical conditions, and treatment goals in advance, helping you and your psychiatrist make the most of your appointment.
Appointments may need to be rescheduled if required forms are not completed before the scheduled visit.
Insurance and Benefits
Goldstone Psychiatry participates with several insurance plans and also offers private-pay services. Insurance participation does not guarantee that every service will be covered.
Your copayment, coinsurance, deductible, and other patient responsibility are determined by your insurance plan. Any benefit information provided before your appointment is an estimate and is not a guarantee of payment by your insurer.
Patients are responsible for charges not covered by their health plan.
Credit Card Authorization
A valid credit or debit card must be kept securely on file to schedule and maintain an appointment.
For patients using insurance, the card may be charged for applicable copayments, coinsurance, deductibles, late-cancellation fees, missed-appointment fees, and balances remaining after the insurance claim has been processed.
For private-pay patients, payment may be collected before or at the time of service.
Payment information is stored through our secure, HIPAA-compliant practice-management system.
Cancellation and Missed-Appointment Policy
We understand that schedules sometimes change. If you need to cancel or reschedule, please provide at least 48 hours’ notice so the appointment can be offered to another patient.
Cancellations made with less than 48 hours’ notice and missed appointments are subject to a $100 fee. These fees are generally not covered by insurance.
Telepsychiatry Appointments
For virtual appointments, you must be physically located in a state where your clinician is licensed at the time of the visit. Please join from a private, stationary location with a reliable internet connection.
For safety purposes, your clinician may confirm your physical location and an emergency contact at the beginning of the appointment.
